Documents needed to COMPLETE Enrollment

  • Student's Birth Certificate
  • Student's Social Security Card
  • Immunization Records
  • Proof of Custody (if applicable)
  • Proof of Residency: 3 documents with your current street address. (rental agreement, utility bill, voter id card, etc.)
  • Guardian’s Driver’s License (with current street address)
  • Grades and Proof of Current Grade Level from Previous School
  • Standardized Test Scores
  • Behavioral Records
  • Attendance Records
  • Other Information Effecting Placement



Hope—Hope is important to beginning any venture.  If we see the possibility for success, then we have a starting point.  Our 5th Grade Academy is our House of Hope. 

Optimism—Optimism moves us a step closer to reaching our goals.  We no longer just see the possibility…we now see the probability of climbing to higher heights and reaching greater success.  Our 6th graders are the House of Optimism.

Unity—Unity is important because we know that together we can do mighty things that could never be accomplished by one or two alone.  Our 7th graders are the House of Unity.

Spirit—Spirit is the fire and motivation that propels us forward and reassures us that we can do all things with proper preparation and drive.  Our 8th graders are the House of Spirit.

Excellence for Everyone—Excellence for Everyone is the goal of the faculty and staff as the teachers and administrators help students to tap into inner greatness while pursing their own quests for professional and personal excellence.  All of Mansfield Middle School is the House of Excellence.


Our HOUSE will promote increased awareness of the primary components needed for success in Middle School (attendance, behavior, and core curriculum).  Our HOUSE will also assist students in accepting more ownership.  Our HOUSE will develop leadership skills in many of our students.  Our HOUSE will use friendly competition to build a “can do” spirit, increase a sense of pride in doing well, and cultivate a strong family of students who work hard and help each other to excel. Our HOUSE of Wolverines will set high standards and reach beyond those standards to demonstrate excellence.
